Sidney Cero Eisgruber, OTR, CMC

Sidney’s passion to help older adults began in high school where she worked as a nurses aide at a local nursing home. She attended Indiana University, graduating with a bachelor’s degree in Occupational Therapy and worked in West Virginia and southern California before settling back home again in Indiana.
She practiced in the field of geriatrics for most of her occupational therapy career, working mainly in the long term care arena.
Prior to starting her family, she served in various capacities such as a Staff Occupational Therapist, Rehabilitation Manager, Clinical Consultant, and a Rehabilitation Consultant.

Sidney is a member of the Indiana Occupational Therapy Association and was appointed in 2014 by then-Governor Mike Pence to serve on Indiana’s Occupational Therapy Committee. She enjoys serving on numerous boards in the community.
With her 30 years of professional clinical experience, becoming a geriatric care manager was a perfect match to help the aging population maximize their quality of life and functional ability.
Sidney is a member of the National Academy of Certified Care Managers and is an Advanced Professional Member of the Aging Life Care Association (ALCA). She is a member of the Indiana Occupational Therapy Association (IOTA) and serves on the Steering Committee of the Hoosier Falls Prevention Coalition.
She serves the Indianapolis area and surrounding counties.



Christine Fogel, RPT
Assistant Care Manager
Christine graduated from Northwestern University in Boston with a Bachelor of Science degree in physical therapy. After working inpatient rotations at University Hospital in in Boston, Christine and her husband, Dr. Rich Fogel, moved to Indianapolis.
She worked in inpatient acute rehab at Hook’s Rehabilitation Center and then transitioned to Brookview Care Center where she worked in subacute rehabilitation as a staff PT, eventually becoming the Director of Rehabilitation.

After having kids, she worked part time so she could keep up her skills while devoting time raising her three kids. In Christine’s free time, she enjoys watching her grandson Lane, hiking and taking in the beauty of Colorado. She enjoys being an Assistant Care Manager because she gets to meet people exactly where they are while utilizing creativity to address solutions and solve problems.
Sidney feels so fortunate to have someone like Christine on her team, “she really cares about people and is an amazing problem solver!”
